Service System Coordination

Children's Centre Thunder Bay offers help to streamline and coordinate services for parents and caregivers seeking support for children with multiple needs. Service System Coordination is available in cases where additional services are required due to the number and/or complexity of children's needs. The Service System Coordinator works in partnership with families and caregivers to create a plan that addresses specific child needs.


This service is offered independently and in collaboration with the children’s service system including Children’s Centre Thunder Bay, Dilico Anishnabek Family Care, and North of Superior Counselling programs. This network of partners strives to evaluate and prioritize services to fill service gaps where possible, and to develop new approaches to achieve better results for children and youth.


Children and youth under the age of 18 years old with needs that are beyond the available services and supports offered within the District of Thunder Bay are eligible for this service. A documented history must be available to demonstrate that all other available resources have been exhausted. Eligibility criteria must also include a diagnosis of more than one disability, including those listed below. The accompanying difficulties must require multiple services and environments.


  • Developmental Disability (I.Q. below 70)
  • Mental Health Diagnosis
  • Physical Disability
  • Autism Spectrum Disorder
  • Medically Fragile Technologically Dependent
  • Acquired Brain Injury
  • Rare/Extreme Condition


Case conferences involving various community partners along with the family are common with this service. It is an effective process, allowing multiple service providers to collaborate to develop, synchronize, and manage the implementation of an integrated service plan to meet the identified needs of children and youth and their families in order to support goal achievement. Further referrals and access to additional services may be recommended as part of this process.
