Self Referrals Welcome
Referrals are accepted for Counselling and Therapy. Completed referrals can be sent through the online referral form below, or by completing the printable referral form.
Referrals for our Live-In Treatment Program, Preschool Speech and Language, Blind/Low Vision, and Infant Hearing Program are completed by telephone only.
Referrals can also be made by phone to Intake at 1-855-486-5037 or 807-343-5046. Please note that summer hours for Children's Centre Thunder Bay are in effect from the end of June until the first week of September.
Printable referral forms are available by opening the referral form and right clicking to reveal a pop-up window where the option to print will appear and can be chosen. Printed and completed referral forms can be faxed to 807-345-0444, or mailed to:
