Client Rights & Responsibilities
What are your rights?
As a client with the Centre, you have the right to:
- Be treated with dignity and respect and without discrimination
- Get help in the language that you speak
- Confidentiality and privacy
- Receive services in a safe and secure environment
- Take part in the planning and review of services
- See your records/file and have copies of your reports
- Tell us who we can give information to by signing the “Consent to Release or Obtain Client Personal Health Information” form
- Talk with us if you feel that any decision or service is not right for you
- To change, limit or withdraw your consent
- Complain about services
We will also:
- Ask you to give verbal or written consent for service
- Ask you about any cultural, spiritual or diversity issues that may be important to our work with you
What are your responsibilities?
- To attend services
- To let us know at least 24 hours in advance if you have to cancel an appointment
- To let us know if the service you are receiving is not helping, or the service needs to be changed
- To please provide us with feedback and input on services you have received at the Centre
